Care that fits your lifestyle.
The Your Choice plan provides you and your family with access to quality care from the large, national UnitedHealthcare nationwide network of 1.8+ million physicians and health care professionals, and 5,600+ hospitals.1 You don’t need a Primary Care Provider (PCP) on file and no referrals are required for specialists.
You get to decide.
Seeing how much youʼll owe before care is pretty powerful, and it’s a unique benefit of the Your Choice medical plan option. Visit the Surest app or Benefits.Surest.com to search for care by symptom or condition to find in-network providers, copays, and treatment options that fit your health need, lifestyle, and budget.
Virtual care—available (almost) anywhere, anytime.
Making quality care more convenient matters, so the Your Choice plan offers access to dozens of on-the-go virtual care providers and programs using a phone, tablet, or computer from the comfort of your home, car, or (pretty much) anywhere.
Get virtual care for nonemergency conditions such as allergies, ear infections, colds, or pinkeye.
The plan has a $0 copay for virtual acute, primary care, and mental health services and low-cost copays for virtual specialty care.
You can quickly find virtual care options using the Surest app or website when you search “virtual visit”.
Be aware your primary care physician (PCP) may offer virtual or telehealth services, which often differ in pricing from the virtual care options offered by the Your Choice plan.
Taking care of you (and your dependents).

Even if you’re feeling great, seeing your doctor once a year is a smart idea—to keep up with annual exams and recommended screenings to help find little problems before they turn into bigger ones. As part of your coverage, your preventive care is covered 100% when you see an in‑network provider. What’s more, approximately 98% of FedEx employees are within an estimated 15-minute drive of a UnitedHealthcare primary care provider (PCP).**
If you need help getting started, a Choose Well Care Advocate can help you find a doctor, review recommended screenings, and more.

When planned or unexpected medical expenses happen, it’s nice to know you’ve got money set aside. Having a Health Savings Account (HSA), Health Reimbursement Account (HRA), or Flexible Spending Account (FSA) with Optum Financial® helps you do just that—while taking advantage of tax savings.
Flexible Spending Account (FSA)
With the Your Choice plan you can contribute to a Health Care FSA:
- Any eligible medical, mental health, and substance use expenses will be paid from the FSA first.
- The Health Care FSA can also be used for dental, vision, and pharmacy expenses, unlike the HRA.
Getting started
If you haven't already, activate your Optum Financial account through the online portal at https://myoptumfinancial.com/fedex
Health Savings Account (HSA)

Health Reimbursement Account (HRA)
