Plan pricing

Surest is a no deductible, copay plan. The Surest app is designed to show a single price (not an estimate) for care so you know what you’ll owe before a visit.

Deductible
$0
A deductible is the amount you have to spend out-of-pocket before your coverage goes into effect, a sort of down payment on your health care. By getting rid of the deductible - making it $0 and giving consumers more informed choices - Surest members pay less in out-of-pocket expenses than they would typically spend trying to reach a deductible.
Coinsurance
$0
Coinsurance is the percentage of the medical bill you pay – typically this is after you’ve met your deductible. The insurance plan pays the remainder of the bill. It can be difficult to know what your bill will be given it is a percentage of a cost that is unknown until the bill arrives. The Surest plan does not use coinsurance.
Individual Out-of-pocket Limit
{{oopIndividualMax}}
The most you could pay during a coverage period (usually one year) for your share of the costs of covered services. After you meet this limit, the plan will usually pay 100% of the allowed amount. This limit helps you plan for health care costs.
Family Out-of-pocket Limit
{{oopFamilyMax}}
The most your family could pay during a coverage period (usually one year) for your share of the costs of covered services. After you meet this limit, the plan will usually pay 100% of the allowed amount. This limit helps you plan for health care costs.

About the Surest plan

Will my providers be covered?
Your plan gives you access to the broad, national UnitedHealthcare network of doctors, hospitals, and clinics. If you need help finding a provider, check out Join.Surest.com to test-drive the plan. Still have questions? Get in touch with an expert.
What services are covered?

Your plan covers numerous common care needs—including:

  • Preventive care (annual checkup, some health screenings, vaccinations)
  • Primary and specialty care
  • Hospital services
  • Urgent and emergency services
  • Most diagnostic testing (ultrasounds and bloodwork)
  • Mental and behavioral health services
  • Maternity care
  • Cancer care
  • Treatment for chronic conditions
  • Substance use disorders and more

For details on the services included in your coverage, refer to your plan materials.

How can I find the cost for care?
Once you’re a member, simply go to your Surest account, go to the search tool, and search for care by condition or symptom (for example, “pink eye” or “my head hurts”). After you select the type of visit you want, you’ll see a list of provider copays (prices). Copays may vary for the same service/procedure depending on who the provider is and the location where it’s being provided. When you check prices before seeking or scheduling care—you have the power to potentially save money. You can test-drive the plan at Join.Surest.com to see how it works.
Is the price listed final?
The Surest app and website are designed to show a single, upfront price (copay), not estimate, in advance to help you choose where/how to get care. A copay is often paid at the time of service, or the provider may bill you.
How are copays lower for better care?
Surest assigns copays based on a number of factors, including providers (doctors, nurse practitioners, and physician assistants) who achieve better patient health outcomes, have lower rates of complications, and use resources more efficiently. When providers are evaluated as high-value, Surest assigns a lower copay. Surest also evaluates locations (hospitals, Ambulatory Surgical Centers, and clinics) for quality, which helps inform the copays we set.
Is it true I don’t have a deductible or coinsurance ... ever?
With the Surest plan, there’s no fog around health care costs and what’s owed. There’s no deductible and no coinsurance. Period. You have coverage from day one, no calculations required.
