To figure out the cost of care (your copay), use the Surest app or website to search for care—or connect with Member Services for help.

When you open the Surest app or website, use the search feature to help you find a provider, including the copay amount.

You don't need to know a bunch of medical terms to find the care you need. By entering common words, symptoms, or conditions, such as "my back hurts" or "knee pain," you can easily find health care services and get your copay price.

Providers considered high-value—based on the quality, efficiency, and overall effectiveness of the care they provide—will have lower copay prices.

Understanding what’s included.

Before making an appointment, be sure to check the “what's included” section. This will help you understand which services are covered under a single copay and when a second copay might apply.

For instance, office visits include standard labs and X-rays. If you go to the doctor for a sore throat, and they think you may have strep, the strep test is covered by your visit copay.

During your appointment, if you need follow-up care, the Surest app can help you find the right location. With your provider, pull up the app to compare options based on your preferences. You’ll know the copay price before you even make the visit.
