How to find the price (copay) for care

One of the unique features of the Surest health plan is using the Surest app or website to find a single copay for care before receiving it. To find a copay:

Search by symptom/condition → type of visit → provider and location = Copay

If you change the service, location, or provider, then you’ll need to start a new search to find the price.

If you can’t find a service or specific location or provider, contact Member Services online via chat or email, or call the number on the back of your ID card.

To get to a single copay, it’s important to start a search with what you need and then select a provider and location. Copays may vary for the same health service based on who you see and where you go for care. This means the price you see for a service at one location might not be the same at another location, even if itʼs the same doctor.

Value-based copays

Surest evaluates providers and locations on past performance using a broad set of factors, including effective and cost-efficient criteria, which inform its copays. Those that performed better are considered high-value and often priced lower. See more about our copays.

Surest also bundles health services that often occur together and prices them accordingly so you can see what’s included in a single price. By grouping services—like combining tests or services that accompany a major medical procedure into one price—you can more easily know a complete price for a health care event before a medical bill is processed.

Before going to get care with any provider, search the Surest app or website to see what your options and costs are so you can plan ahead. You can also use the app in the doctor’s office so you can know right away if a recommended specialist or treatment is in-network and the copay. If you have questions about copays, you can connect with Member Services via chat, email, or by calling the number on the back of your Surest member ID card.
